Luckily she was such a pretty and desirable lady,
so not long she had remarried to another man.
She is now married to her new husband for 4
years without giving him any child.
Now she is desperate and in great pain.
I can assure you that this true story is not only
that of Ann. I have heard this story continuously
in my practice for many year. Sometimes I wake
up in the morning from bed with the thoughts and
sorrows of these women in my head.
WHAT IS INFERTILITY
In this book I will try not to bore you with so many
long stories, definitions and explanations. However
I will be glad if you can be patient and try and
follow me as I make all things clear.
FERTILITY
Fertility is the natural capability of giving life. As
a measure, “fertility rate” is the number of children
born per couple, person or population.
Infertility is defined as the inability of a couple to
conceive after 12 months of unprotected sexual
intercourse.
However if the lady is 30 years and above, the
definition changes to the inability of a couple to
conceive after 6 months of unprotected sexual
intercourse
Women’s fertility peaks between the ages of 22 to
26, and often declines after 30: a typical 30 year
old woman has 12% of the ovarian reserve she was
born with, and has only 3% at age 40. With a rise
in women postponing pregnancy due to
educational pursuit or carreer ,this has created
infertility problem.
Below are researched statistics of women trying to
get pregnant without using fertility drugs or
invitro fertilization
[1] At age 30, 75% will get pregnant within one
year, and 91% within four years.
[2] At age 35, 66% will get pregnant within one
year, and 84% within four years.
[3] At age 40, 44% will get pregnant within one
year, and 64% within four years.
The above figures are for pregnancies ending in a
live birth and take into account the increasing
rates of miscarriage in the aging population.
According to the March of Dimes, “about 9 percent
of recognised pregnancies for women aged 20 to 24
ended in miscarriage. The risk rose to about 20
percent at age 35 to 39, and more than 50 percent
by age 42″.
Birth defects, especially those involving
chromosome number and arrangement, also
increase with the age of the mother.
When a patient comes to our clinic for
consultation, if she is newly married for up to six
months, never had any pregnancy before, but is
over 30 years of age, she will be assumed to be
infertile.
Doctors recommend that women over 30 who have
been unsuccessful in trying to conceive for more
than 6 months must undergo some kind of fertility
testing.
From the above details you can see that the rate of
pregnancy goes down with age.
Although women can get pregnant anytime during
their menstrual cycle,
the best days to get pregnant are 2 days before
and after ovulation because
the sperm can only survive 2-3 days(sperm may
survive up to 6 days) in
the uterus after ejaculating into the vagina.
THE NATURAL PROCESS
- Men produce sperm as soon as they reach
puberty and at any time of the
month, but their sperm quality and quantity may
dip occasionally, depending on their internal cycle,
therefore it is easy to understand that some
women can conceive with thier partner at the age
of 95. - The female is born with a finite number of eggs
in their ovaries. When females reach the age of
puberty, every month some of the eggs in their
ovaries mature and are extruded and only one can
get into the Fallopian tube for fertility, the rest
degenerate and die. When the eggs are less than a
certain number in the ovaries, she enters the
menopause state.
SUBFERTILITY: is used to describe gradations
between normal fertility and sterility, often used
interchangeably with infertility.
FECUNDABILITY: is the pregnancy rate from one
menstrual cycle. The normal rate in humans is
20%. Seventy-five percent of normally fertile
couples are expected to have conceived in six
months and almost 100% by one year.
STERILITY: is the absolute inability to procreate:
an absent uterus in women, absent testes in men.
In years past, a woman with blocked fallopian
tubes or man with an obstructed vas deferens
would be sterile. But with assisted reproductive
technology (ART), this is no longer the case.
THREE BASIC QUESTIONS IN INFERTILITY
There are really three basic questions that have to
be answered when doctors try to determine why a
woman is having problems getting pregnant.
[1]Is she ovulating?
[2]Is there a clear passage from the ovary to the
uterus?
[3]How old is she?
A similar set of questions has to be answered in

problems specific to females. For example, if a
couple is struggling to achieve pregnancy and the
male has adequate sperm count, motility, and
shape, but the woman has polycystic ovarian
syndrome, then their inability to conceive is likely
due to female factor infertility.
FEMALE FERTILITY PROBLEMS
There are several conditions that contribute to
female factor infertility, including uterine and
pelvic abnormalities, secondary infertility,
polycystic ovarian syndrome, and hostile cervical
mucus. It is important to understand, however,
that infertility, whether male infertility or female
infertility, is not the same thing as sterility .
Conception and successful pregnancy are possible
in many cases, likewise secondary infertility (the
inability of a couple to conceive after having
already achieved a successful pregnancy or
pregnancies) can often be treated.
